This form is intended for use by an adult who will be travelling internationally, or by air, with a child, when the child is not travelling with both natural parents, (i.e. only one parent, or some other family member, teacher, coach/chaperone is travelling with the child). Please note that an additional form is required if the child is travelling with different people for the outbound/return journey.
