Child Travel Permission

This form is intended for use by an adult who will be traveling internationally, or by air, with a child, when the child is not traveling with both natural parents. ( ie. only one parent, or some other family member, teacher, coach/chaperone is traveling with the child). To be valid, the document must be notarized.

To use this online form, please fill in all of the required fields, and submit. A copy will be sent electronically to Greenway Legal Centre, where it will be available to be notarized. You will also receive a copy via email in the event that you cannot attend the offices of Greenway Legal Centre and have to have the document notarized elsewhere.
